The student must be accompanied by the parent in filing this application for transfer or withdrawal

Present the accomplished form to the Registrar's Office for approval. Allow at least a week for processing and release of any refund, if applicable

Allow two (2) days for release of certificate of enrolment or certificate of good moral character by the Office of the Principal.

Allow at least a week for the release of transfer credentials by the Registrar's Office.
